Community Programs


Innovation Care Partner’s has established many community partners to assist in achieving quality outcomes for our patients. We have established preferred provider agreements with post-acute organizations to ensure our patients are supported through out the continuum of care. ICP is a dedicated to providing learning environments for patients. We hold a quarterly “Chronic Disease Self-Management Program” for patients to attend and improve their skills in managing chronic illnesses.

The ability to access services for people and families is a key mission for Innovation Care Partners (ICP). Using 2.1.1. AZ website is a quick and easy way to find community resources for yourself, friends & family. Through the use of 2.1.1 AZ website, all Arizonans are easily connected to available health and human services in their communities. This “Community Information and Referral Services” website transforms lives by linking individuals and families to vital community services throughout Arizona. Our ICP Care Coordinators are specially trained on knowing these community services as well as other health related resources.
